OK, I am feeling VERY lazy today, besides being swamped with work. (We are starting a new account Thursday morning and another on Monday and they only told us about it today) So...... to the point of this thread. I need to design a Daily Report format for my Field Supervisors to complete each day. You know, post checks, how many miles, who was on duty, uniform appearance etc..... before I bust my hump with everything I was hoping that one of our fine members might have a generic form I could use/update for my immediate needs. Thanks all!

Has anyone used Adobe LiveCycle for forms? My only ... about the thing is that it saves files as XML.Some Kind of Commando Leader

Nathan, apparently Omni Form is pretty decent. A small police department uses it for all their reports. You can customize it with drop down windows and all.Originally posted by N. A.
