It's called Counterterrorism Strategies for Corporations: The Ackerman Principles, by Mike Ackerman. The title is a little misleading as it is really mostly about EP. Lots of real-life examples and interesting stuff like this:
"Assailants armed with 9-mm pistols, if able to put their rounds into a tight circle, can breach the windows of a level three armored car in about six seconds...and it is wise to bear in mind that the polylaminate security glass used in armored vehicles deteriorates over time, especially in warm sunny climates. The glass of an older armored vehicle that has not been recently reconditioned could probably be breached in even less time." (p. 192)
There are multiple useful checklists in the back section of the book also.
